By booking an appointment with AJCBEAUTYCARE LLC, you confirm that you have read, understood, and willingly consent to the following terms and conditions:

• I consent to having eyelash extensions applied to my natural lashes and/or brow services performed, including the application and/or removal of products, by a licensed professional.

• I understand and agree to follow all aftercare instructions provided. I acknowledge that AJCBEAUTYCARE LLC is not responsible for any adverse results, reactions, or issues that occur due to failure to follow aftercare instructions, and I accept full responsibility for any such outcomes.

• I understand that, although rare, there are risks associated with eyelash and brow services, including but not limited to eye or skin irritation, allergic reactions, discomfort, or sensitivity.

• I understand that due to the natural lash growth cycle, shedding, and normal wear and tear, maintenance appointments are required. Touch-ups are typically recommended every 2–3 weeks to maintain the appearance of eyelash extensions.

• I authorize AJCBEAUTYCARE LLC to securely store my card on file and understand that my card will be charged in the event of a no-call, no-show, or late cancellation, in accordance with the business cancellation policy.

• I understand that photos and/or videos may be taken during or after my appointment. I grant permission for AJCBEAUTYCARE LLC to use these images for marketing, promotional, educational, and social media purposes on any platform, unless otherwise stated in writing prior to my appointment.

• I acknowledge that by booking an appointment, I agree to all policies and understand that these policies are subject to enforcement.

HEALTH & SAFETY ACKNOWLEDGMENT

By booking, I acknowledge and agree to the following:​

• I understand there are inherent risks associated with receiving personal services.

• I confirm that I am not experiencing symptoms such as fever, cough, shortness of breath, body aches, nausea, or loss of taste or smell at the time of my appointment.

• I confirm that I have not knowingly been in close contact with anyone experiencing contagious illness symptoms within the past 14 days.

• I understand that no additional guests or children are permitted at my appointment unless they are being serviced.

• I agree to notify AJCBEAUTYCARE LLC if I experience illness within 14 days following my appointment.

RIGHT TO RESCHEDULE OR CANCEL

• I understand that AJCBEAUTYCARE LLC reserves the right to reschedule or cancel appointments due to unforeseen circumstances, including but not limited to illness, emergencies, or severe weather.
